Renewal of Registration (Requisites)---->Download Form

• Three recent passport size photographs with name and signature at the backside.
• Original Delhi Medical Council Registration Certificate.
• Bank Draft of Rs 1000/ (Rupees One thousand only) as non-refundable fee in favour of "Delhi Medical Council" payable at New Delhi.

Provisional (Requisites)---->Download Form

(a) Applicants having passed final MBBS examination from Delhi

• Application is to be submitted in the office of the Delhi Medical Council along with four recent passport size photographs with name and signature at the backside.
• Provisional degree/diploma or provisional certificate of having passed the MBBS examination issued by the Dean/Principal/Registrar of the College/University in original alongwith relevant copies be forwarded with this application.
• Certificate of Date of Birth (Secondary School Certificate or equivalent).
• Bank Draft for Rs. 500/- (Rupees Five Hundred only) in favour of the Delhi MedicalCouncil payable at New Delhi should be submitted with the application as nonrefundable fee for provisional registration.
• Identity proof : Photo Identity Card / Passport / Driving License / Electoral Card.

(b) Applicants having passed final MBBS examination from other states
In addition to requirements mentioned at (a) the following are to be complied with

• Marksheets of I, II, III (Part 1 & 2) professional exams
• No objection certificate from College/University
• Class 10th & 12th Marksheets with passing certificates
• Provisional registration certificate (In case already provisionally registered with other State medical Council/MCI.)

(c) Applicants having passed final MBBS/MD 'Physician' examination from outside India
In addition to requirements mentioned at (a) & (b) the following are to be complied with

• Screening Test Copy
• Degree/Diploma and Mark Sheet of MD ‘Physician’ qualification (if the same in as language other than English then an authentic translation in English)
• Copy of the passport duly attested by a Gazetted officer(from the first page to last page with English transalation of the content in the passport)

All the documents are to be produced in original alongwith a photocopy(in case of category (b)&(c)   attested photocopies)
Application for provisional registration is to be submitted in person

Fresh Registration (Requisites)--->Download Form

In case of registration for the first time who are permanently registered with Other State Medical Council/MCI

• Four recent passport size photographs with name and signature at the backside.
• State Medical Council/Medical Council of India Registration Certificate with MBBS Qualification
• MBBS Degree/Post-Graduate Degree/Diploma/Post-Doctoral Degree (Only Degree Certificates issued by the University will be entertained).
• Birth certificate/matriculation certificate/SSC Exam certificate/ School Leaving certificate with Date of Birth.
• Identity proof : Photo Identity Card /Passport / Driving Licence / Electoral Card.

In case of registration for the first time after completion of Internship

(a) Person registered provisionally with Delhi Medical Council

• Four recent passport size photographs with name and signature at the backside.
• Original Internship Completion Certificate. The same shall be returned along with Delhi Medical Council’s certificate of permanent registration.
• Original Provisional Registration Certificate.

(b) Person registered provisionally with other State Medical Councils / MCI
In addition to requirements mentioned at (a) the following are to be complied with

• Identity proof :Photo Identity Card / Passport / Driving Licence / Electoral Card.
• MBBS passing certificate issued by the College/University
• Marksheets of I, II, III (Part 1 & 2) professional exams
• Class 10th & 12th Marksheets with passing certificates

Note : - Application for registration is to be submitted in Person.

Foreign Graduate (Requisites)--->Download Form

I submit herewith original certificates for verification and submit attested copies of the following document/certificates :

• Four recent passport size photographs with name and signature at the backside.
• State Medical Council/Medical Council of India Registration Certificate with MD ‘Physician Qualification
• Degree/Diploma and Mark Sheet of MD ‘Physician’ qualification (if the same in as language other than English then an authentic translation in English)
• Internship Completion Certificate
• Screening Test Copy
• 10th Mark Sheet and passing certificates or equivalent examination.
• 12th Mark Sheet and passing certificates or equivalent examination.
• Birth certificate/matriculation certificate/SSC Exam certificate/School Leaving certificate with date of Birth.
• Copy of the passport in possession duly attested by a Gazetted Officer (From first page to last page with English Translation of the contents in the passport)
• Other evidence in support of my having obtained the qualification which I possess in original

Note :- Application for registration is to be submitted in Person

AdditionalQualification (Requisites)--->Download Form

Delhi Medical Council Registration Certificate.
Additional qualification : Post Graduate Degree/Diploma
Certificate/Post-Doctoral degree. - Two recent passport size photographs. - D.D of Rs.500/- for each additional qualification in favour of "Delhi Medical Council".
Note : Only Diploma/Degree certificate awarded by the University will be entertained
All the above-mentioned documents are to be produced in original alongwith a photocopy.

Good Standing (Requisites)--->Download Form

• A Bank Draft of Rs 1000/- (Rupees One Thousand Only) as non-refundable fee in favour of “Delhi Medical Council” payable at New Delhi .
• Attach a photocopy of DMC Registration Certificate.

Note :- The certificate of Good Standing issued by the Delhi Medical Council will be valid upto six months from the date of issue.

Practice of Foreign Nationals in India--->Download Form

(To be filled in Triplicate along-with enclosures and to be submitted to Delhi Medical Council six months prior to the visit to India or at the earliest.)

No Objection certificate

• An application addressed to the Registrar for issue of No Objection certificate
• Attach a photocopy of DMC Registration Certificate.
• A Bank Draft of Rs 1000/- (Rupees One Thousand Only) as non-refundable fee in favour of “Delhi Medical Council” payable at New Delhi .

Issue of Duplicate Registration Certificate

Application on a plain paper addressed to the Registrar, Delhi Medical Council for issue of Duplicate Registration Certificate along with the following is to be submitted in the office of Delhi Medical Council : -

• Two recent passport size photographs.
• A Demand Draft of Rs. 500/- in favour of Delhi Medical Council payable at New Delhi.
• NCR/FIR with the police intimating about the loss of Delhi Medical Council Registration Certificate.
• Indemnity Bond as per the proforma attached
